
Required Documents for Visa Application
- Visa Application Form
- Each applicant, including children, must submit a separate form.
- The form should be filled in block letters (typed or clearly handwritten).
- The applicant’s name must match the passport details.
- Photograph
- Size: 2-inch x 1.4-inch (45mm x 35mm).
- Full-face view on a white or light-colored background.
- Taken within the last 6 months.
- Valid Passport
- Name and details should match the National Identity Card (NIC).
- Two copies of the passport bio-data page.
- One copy of the alteration page, if applicable.
- Must have at least two blank pages.
- Schedule of Stay
- A detailed day-by-day itinerary of activities in Japan.
- Accommodation Proof
- Hotel booking confirmation or a sponsor’s residence certificate if staying with a host.
- Travel Itinerary
- Round-trip flight reservation (It is recommended not to purchase tickets before visa approval).
- Employment & Financial Documents
- Employed Applicants:
- A letter from the employer stating employment start date and approved leave period.
- Self-Employed Applicants:
- Business registration document (Original).
- Company Bank Statement (Last 3 months, Original).
- Students:
- A letter from the school or university confirming the approved leave period.
- Personal Documents (Mandatory)
- National Identity Card (NIC).
- Birth Certificate.
- Marriage Certificate / Divorce Certificate / Death Certificate (if applicable).
- Affidavit (if there is a name mismatch in official documents).
- Personal Financial Records (Last 3 months)
- Salary Slips.
- Bank Statement & Bank Balance Confirmation Letter (for current and savings accounts).
- Fixed Deposit balance confirmation (if applicable).
- Previous Travel History to Japan (Last One Year)
- Dates and duration of previous visits, if applicable.
Additional Notes
- A request letter must be submitted individually by each applicant, stating they have applied through a travel agent.
- Retired government employees must submit retirement documents.
- Non-government employees should provide a recommendation letter explaining their employment status and source of funds.
- All documents should be sorted according to the checklist.
- Original documents and A4-sized photocopies must be submitted separately.
- Documents in Sinhala or Tamil must be accompanied by an official English translation by a sworn translator.
- Do not use pins, staples, paper clips, or cello tape on documents.
- The standard processing time is 5 working days, but additional scrutiny may extend this period.
- Visa fees are non-refundable, even if the application is rejected.
